O primeiro racional seria reduzir sintomas.

Detection of bloodstream infections in adults: how many blood cultures are needed?
J Clin Microbiol. 2007; 45(11):3546-8

Department of Medicine, MEB 364, Robert Wood Johnson Medical School, New Brunswick, NJ 08901-0019, USA.
Although several reports have shown that two to three 20-ml blood cultures are adequate for the detection of bacteremia and fungemia in adults, a recent study (F. R. Cockerill et al., Clin. Infect. Dis. 38:1724-1730, 2004) found that two blood cultures detected only 80% of bloodstream infections and that three blood cultures detected 96% of episodes. We reviewed data at two university hospitals to determine whether the recent observations by Cockerill et al. are applicable more widely. We assessed all blood cultures obtained from adult inpatients from 1 January 2004 through 31 December 2005 at Robert Wood Johnson University Hospital and Duke University Medical Center. All instances in which > or =3 blood cultures per patient were obtained during a 24-h period were included. The medical records of patients who met the inclusion criteria were reviewed retrospectively to determine the clinical significance of the positive blood culture (true infection versus contamination). Data were analyzed to determine the cumulative sensitivity of blood cultures obtained sequentially during the 24-h time period. Of 629 unimicrobial episodes with > or =3 blood cultures obtained during the 24-h period, 460 (73.1%) were detected with the first blood culture, 564 (89.7%) were detected with the first two blood cultures, 618 (98.2%) were detected with the first three blood cultures, and 628 (99.8%) were detected with the first four blood cultures. Of 351 unimicrobial episodes with > or =4 blood cultures obtained during the 24-h period, 257 (73.2%) were detected with the first blood culture, 308 (93.9%) were detected with the first two blood cultures, 340 (96.9%) were detected with the first three blood cultures, and 350 (99.7%) were detected with the first four blood cultures. Among unimicrobial episodes, Staphylococcus aureus was more likely to be detected with the first blood culture (approximately 90% detected with the first blood culture). There were 58 polymicrobial episodes in which > or =3 blood cultures were obtained. Forty-seven (81.0%) were detected with the first blood culture, 54 (93.1%) were detected with the first two blood cultures, and 58 (100%) were detected with the first three blood cultures. The results of this study indicate that two blood cultures in a 24-h period will detect approximately 90% of bloodstream infections in adults. To achieve a detection rate of >99%, as many as four blood cultures may be needed. The previously held axiom that virtually all bloodstream infections can be detected with two to three blood cultures may no longer be valid but may also depend on the definition of the "first" blood culture obtained

MAIS ALGUNS PENSAMENTOS SOBRE O CORTICUS...
- Maior parte dos pacientes incluídos eram pós-operatórios de emergência;
- Na análise de subgrupo, entre os pacientes que permaneceram hipotensos nas primeiras 30 horas após a entrada no estudo, houve uma redução de cerca de 20% na mortalidade - 11,2% de diferença absoluta (conclusão gerada com um n pequeno, claro, o que faz pensar o que poderia ter acontecido se o estudo tivesse concluído seu planejamento amostral);

Hydrocortisone Therapy for Patients with Septic Shock
Charles L. Sprung, Djillali Annane, Didier Keh, et al.
Background
Hydrocortisone is widely used in patients with septic shock even though a survival
benefit has been reported only in patients who remained hypotensive after fluid
and vasopressor resuscitation and whose plasma cortisol levels did not rise appropriately after the administration of corticotropin.
Methods
In this multicenter, randomized, double-blind, placebo-controlled trial, we assigned
251 patients to receive 50 mg of intravenous hydrocortisone and 248 patients to
receive placebo every 6 hours for 5 days; the dose was then tapered during a 6-day
period. At 28 days, the primary outcome was death among patients who did not
have a response to a corticotropin test.
Results
Of the 499 patients in the study, 233 (46.7%) did not have a response to corticotropin (125 in the hydrocortisone group and 108 in the placebo group). At 28 days, there was no significant difference in mortality between patients in the two study groups who did not have a response to corticotropin (39.2% in the hydrocortisone group and 36.1% in the placebo group, P = 0.69) or between those who had a response to corticotropin (28.8% in the hydrocortisone group and 28.7% in the placebo group, P = 1.00). At 28 days, 86 of 251 patients in the hydrocortisone group (34.3%) and 78 of 248 patients in the placebo group (31.5%) had died (P = 0.51). In the hydrocortisone group, shock was reversed more quickly than in the placebo group. However, there were more episodes of superinfection, including new sepsis and septic shock.
Conclusions
Hydrocortisone did not improve survival or reversal of shock in patients with septic
shock, either overall or in patients who did not have a response to corticotropin,
although hydrocortisone hastened reversal of shock in patients in whom shock
was reversed. (ClinicalTrials.gov number, NCT00147004.)

Dexametasona no Tratamento da Meningite Bacteriana em Pacientes Subdesenvolvidos
Clique no título para o link do artigo.
In a study conducted in Vietnam, Dr. Jeremy J. Farrar and colleagues show that dexamethasone is only effective if given to patients with confirmed bacterial meningitis, not to those with probable disease. In fact, giving the drug to patients with probable disease seemed to increase the risk of death at 1 month.
The findings from the second study, conducted in a sub-Saharan African city with a high HIV prevalence, indicated little or no benefit of using the corticosteroid for this indication. The results of both studies are reported in The New England Journal of Medicine for December 13th.
"Dexamethasone is a very, very cheap drug and probably because there is no industry interest there has remained a question for more than 30 years as to whether it reduces mortality in patients with meningitis," Dr. Farrar, from the John Radcliffe Hospital in Oxford, UK, told Reuters Health.
The results of this study indicate that the drug can produce a "major" reduction in morbidity and mortality if the patient has confirmed disease, Dr. Farrar said.
This last part is important, he emphasized, because in the developing world, TB meningitis is fairly common and can mimic bacterial meningitis, which refers to meningitis caused by non-TB bacteria, usually Streptococcus pneumoniae or Neisseria meningitidis.
The study involved 435 adolescents and adults with suspected bacterial meningitis who were randomized to receive intravenous dexamethasone or placebo for 4 days in addition to a 10- to 14-day course of ceftriaxone. Some of the patients had received antibiotics before inclusion in the study. All of the patients were HIV-negative.
Sixty-nine percent of patients had confirmed bacterial meningitis, 28.3% had probable meningitis, and 2.8% had an alternative diagnosis.
In the overall analysis, dexamethasone therapy did not reduce mortality at 1 month or the risk of death or disability at 6 months. When the analysis was confined to patients with confirmed bacterial meningitis, however, use of the drug reduced 1-month mortality by 57% and the 6-month risk of death or disability by 44%.
As noted, use of dexamethasone for probable bacterial meningitis appeared to increase the risk of death at 1 month. The researchers believe that this is because many of the cases of probable bacterial meningitis actually had TB meningitis, which responded adversely to the drug.
"From this study, if you have an adult patient who is HIV-negative, in whom you confirm acute bacterial meningitis, you should give steroids, even if the patient has had antibiotics before coming to the hospital," Dr. Farrar said. "It also shows how important good diagnostic laboratories are and that by having good laboratories making a quick diagnosis, you can reduce death by a half."
In the second study, which was centered in Blantyre, Malawi, Dr. Matthew Scarborough, also from the John Radcliffe Hospital in Oxford, UK, and colleagues assessed the outcomes of 465 patients who were admitted with bacterial meningitis and randomized to receive intravenous dexamethasone or placebo in combination with intramuscular or intravenous ceftriaxone. In contrast to the first study, 90% of the subjects were HIV-positive.
Dexamethasone therapy did not reduce morbidity or mortality in the study group. Ceftriaxone efficacy was not significantly influenced by its route of administration.
"This trial does not provide support for the routine inclusion of corticosteroids in the management of adult bacterial meningitis in resource-poor areas where pneumococcus is the primary pathogen and where a substantial proportion of patients are likely to have advanced HIV disease," the authors conclude.

Survival of HIV-infected patients in the intensive care unit in the era of highly active antiretroviral therapy
S J Dickson1, S Batson2, A J Copas3, S G Edwards4, M Singer2, R F Miller3
Background: Several studies have described improved outcomes for HIV-infected patients admitted to the intensive care unit (ICU) since the introduction of highly active antiretroviral therapy (HAART). A study was undertaken to examine the outcome from the ICU for HIV-infected patients and to identify prognostic factors.
Methods: A retrospective study of HIV-infected adults admitted to a university affiliated hospital ICU between January 1999 and December 2005 was performed. Information was collected on patient demographics, receipt of HAART (no patient began HAART on the ICU), reason for ICU admission and hospital course. Outcomes were survival to ICU discharge and to hospital discharge.
Results: 102 patients had 113 admissions to the ICU; HIV infection was newly diagnosed in 31 patients. Survival (first episode ICU discharge and hospital discharge) was 77% and 68%, respectively, compared with 74% and 65% for general medical patients. ICU and hospital survival was 78% and 67% in those receiving HAART, and 75% and 66% in those who were not. In univariate analysis, factors associated with survival were: haemoglobin (OR = 1.25, 95% CI 1.03 to 1.51, for an increase of 1 g/dl), CD4 count (OR = 1.59, 95% CI 0.98 to 2.58, for a 10-fold increase in cells/µl), APACHE II score (OR = 0.51, 95% CI 0.29 to 0.90, for a 10 unit increase) and mechanical ventilation (OR = 0.29, 95% CI 0.10 to 0.83).
Conclusions: The outcome for HIV-infected patients admitted to the ICU was good and was comparable to that in general medical patients. More than a quarter of patients had newly diagnosed HIV infection. Patients receiving HAART did not have a better outcome.
Economic Implications of An Evidence-Based Sepsis Protocol
Economic Implications of An Evidence-Based Sepsis Protocol: Can We Improve Outcomes and Lower Costs?
Shorr AF, Micek ST, Jackson WL, Kollef MH
Crit Care Med. 2007;35:1257-1262
Summary
Severe sepsis and septic shock together comprise a common and lethal disease, occurring in 10% to 25% of all patients in intensive care units, with approximate mortality rates of 40% and 60%, respectively.[1,2] With the recent development of various potential therapies for severe sepsis and septic shock (eg, early goal-directed therapy, recombinant human activated protein C, corticosteroids, glycemic control), a new era has emerged for managing these complex patients. Clinical care is moving toward evidence-based protocols that apply the evidence from these studies[1,2] to optimal patient management. Although prior studies have shown that these protocols, such as those advocated by the Surviving Sepsis Campaign,[3] have salutary effects on survival and organ dysfunction,[4,5] they have not been tested economically. The authors of this study sought to determine whether the application of a severe sepsis management protocol was cost-effective in their institution, a tertiary-care academic hospital. Data were collected before and after implementation of a severe sepsis protocol; outcomes were assessed and hospital costs were estimated. Survival increased after implementation of the protocol (from 52% to 70%) and hospital length of stay was reduced by an average of 5 days. Median total hospital costs per patient were less after the implementation of the sepsis protocol ($16,103 vs $21,985), and remained less even when considering only the survivors.
Viewpoint
This study provides an important piece of information for both physicians and institutions to determine how best to optimize the care of patients with severe sepsis. While the true costs of caring for patients with sepsis may vary across institutions, these data suggest that institutions may actually reduce expenditures while saving lives. Many physicians and institutions are already in the process of implementing protocols for the management of these patients, and with the data from this study, even more should be willing to do so. These data may be of most importance for indigent care hospitals where containment of costs is often paramount in order to ensure care for all. At this point, it seems prudent for each hospital to proactively consider how best to manage their sepsis patients, and to strongly consider both the healthcare and financial advantages of protocolized sepsis care.

Papel do sTREM-1 no diagnóstico de pneumonia
Gibot S, Cravoisy A, Levy B et al. Soluble Triggering Receptor Expressed on Myeloid Cells and the Diagnosis of Pneumonia. N Engl J Med. 2004; 350:451-8.
